We performed a pilot study in a small cohort of patients with epilepsy to test the feasibility and accuracy of a new intra-operative imaging method, called "BrainNavigator," which is based on a newly developed multi-channel intra-operative MRI. The brain regions with a neuronal activity increase and a possible epileptic onset zone were determined by using a new software that performs automatic pre-processing of fMRI data. After craniotomy, we performed a combined neuronavigation (BrainNavigator) and intra-operative electrocorticography (ECoG) with two ECoG electrodes in each patient. In all patients, we found an ECoG signal change in relation to the electrode positions on the brain surface as indicated by the BrainNavigator. In seven patients, the BrainNavigator was able to detect and localize the epileptic onset zone by using a combination of the signal intensity changes of the ECoG signals, whereas in two patients the exact localization of the SOZ could not be achieved. Our data support the feasibility of a combined neuronavigation and intra-operative ECoG.
